Dear home-based entrepreneur and home-based advocate:

It was announced a few weeks ago that I won the U.S. Small Business Administration's Home-based Business Advocate of the Year (2003) award. I would like to take this opportunity to speak on your behalf to our elected and appointed leaders and the media in order that we create some changes in policies and programs that will benefit us. In order to do this I am reaching out to you, the Home Based Business Council members in 28 States, the regional Home-Based Business Advocates, and home-based community leaders. I would like for you to complete a very simple survey. The purpose of the survey is to gauge the interests/concerns that you have regarding a few issues confronting us. I also ask that you distribute this survey to your friends, associates, and members who are home-based. If you were not a direct recipient of this e-mail survey from me (chansen@hbbc.org) please indicate the e-mail address or name of the person who referred this survey to you. When the results are in and tabulated a report will be issued to the US Small Business Administration, the media, and the regional advocates for their distribution. All answers to this survey will be kept in strictest confidence and your name will NOT be released to ANYONE without your specific permission. While a better picture can be drawn from this survey if all fields are completed, you must complete all fields not marked (optional).

At this time the survey is available only as a text file.  You may therefore complete it using any word processing program or even copy and paste it into your e-mail program for completion.
